Initial Intake Forms, and all other clinical/practice documents are requested to be scanned or digitally completed and emailed
Areas of Expertise
Dr. Kho does not accept insurance payments and is a cash only practice.
Payment is due at the time of service.
Payment can be in the form of cash, check or credit card (HSA cards included).
